This patient is a 22 year old female from Oneida, NY. She was concerned with excess fat along her flanks and abdomen. She had fat transfers to the buttocks and also extensive lipo-sculpting of her abdomen, flanks and waistline. Often multiple procedures are done at the same time. It can take a number of months for the swelling from lipo-sculpture to go away. In these post-operative photos it shows results after just three months showing profound improvement in the contour of her abdomen and flanks.

This is a 58-year-old male from the Bronx, NY, is 6'2" tall, weighing 180 pounds who was interested in definition of the abdomen. He wanted to really see his six-pack in his abs. The patient did not really work out. He was in relatively good health but again was not very active at the gym and wanted more definition therefore he underwent abdominal etching where we sculpted the abs to define the six-pack muscles as well as the oblique muscles. He also had liposuction of the flanks to further define the waist. The results at three months show excellent definition of the abs with sculpting and etching of the abdominal musculature.

This is a 60 year old female from Ballston Spa, NY who had a large recurrent ventral hernia. She had a left side abdominal hernia repaired with synthetic mesh. She had complications requiring removal of the synthetic mesh that became infected. As a result she developed a massive recurrent ventral hernia causing significant pain and discomfort.The post op photos show results at 4 months after the patient had a complete reconstruction of her abdominal wall with component seperation, muscle advancement flaps and reinforcement of the repair with biological meshes. On the CT scan images, notice the very large herniation with bowel protruding from the defect. On the post-operative CT image, you can see how the abdominal wall has been completely reconstructed and restored with the abdominal contents back in the abdominal cavity.

This is a 53-year-old male who presented with a massive recurrent ventral hernia. As you can see on the preoperative photographs, the patient had a very large midline hernia with extensive amounts of small bowel protruding through the hernia into the subcutaneous area. The patient had a very prolonged complicated history with diverticulitis for which he had small bowel resection and a complicated course after which he developed this very significant hernia which failed previous attempts at repair. In order to fix this patient's hernia, we placed a tissue expander on the left side of the abdomen. This allowed us to expand the muscles thereby creating more muscle that could be advanced and the hernia was therefore closed without destruction of any normal anatomy and the patient had a very solid repair. His core has been reestablished. As you can see in the postoperative photos at three months, he has a very well healed hernia. He has no more protruding bowel and he has resumed most of his normal activities and even playing golf.

This is a 45-year-old male who presented with severe recurrent massive ventral herniation where all of his abdominal content had eviscerated and was essentially outside of his abdomen. The patient had multiple abdominal surgeries with colon resection. He had a very complicated postoperative course with infections and evisceration meaning that the entire content of his abdomen, small bowel and large bowel had eviscerated and was outside of his abdomen. The patient had to undergo extensive skin grafting in order to provide coverage of his intestines. As you can see, on the preoperative photographs the patient has a very wide herniation of the abdomen. The entire content of the abdominal cavity is essentially on the outside covered by a skin graft. In order to start this complex reconstruction first the patient had to lose about 25-30 pounds. We then proceeded to place tissue expanders. These were rectangular tissue expanders placed in a pocket between the internal and external oblique. In the photos showing the expansion, you can see the scars in the subcostal margin where the expanders were placed. These were inflated to about 1,200 cc on each side thus stretching the muscle and creating new tissue that would allow us to reconstruct the patient's abdominal wall and more importantly replace all of the bowel and intraabdominal contents back into the abdominal cavity. The postoperative photograph shows the patient at six months after the reconstruction and repair of the abdominal wall had been performed. During this procedure, the expanders were removed. These were in place for about eight to ten weeks. They were removed and the muscles that were stretched were advanced and used to reconstruct the abdominal wall. The skin graft was removed from the bowel and all of the bowel content was replaced inside of the abdomen. The patient now has a stable strong reconstructed abdomen with his bowels in the abdominal cavity where they belong and he has had no further recurrence of his hernia.

This is a 52 year old gentleman who had a lap band procedure in February 2008. The patient is 5 feet 9 inches and weighed 378 lbs and presented at 218 lbs having lost over 160 lbs. He obviously had significant concerns with extensive excess skin and fat along the abdomen and flanks and also along the upper thighs. The post-operative photo shows him at three months after a body lift which included a full abdominalplasty, thigh lift and buttock lift. Notice the excellent contour of the abdomen and definition of the abdominal muscles as well as sculpting of the waistline and improvement in the thighs as well. The after picture also shows a portion of the scar just to show that it has healed very well in three months and will be covered completely just wearing normal underwear.
